Comparative Analysis of Selective Androgen Receptor Modulators: Ligandrol (LGD-4033) vs. Testolone (RAD-140)
Abstract
Selective Androgen Receptor Modulators (SARMs) were developed to provide the therapeutic benefits of androgen replacement therapy—such as increased muscle mass and bone density—while minimizing the adverse effects associated with conventional anabolic-androgenic steroids (AAS), specifically prostate hypertrophy and erythrocytosis. Ligandrol (LGD-4033) and Testolone (RAD-140) are two of the most studied non-steroidal SARMs. While LGD-4033 has undergone more extensive clinical evaluation in healthy human subjects for muscle wasting, RAD-140 has been characterized primarily in preclinical models for its potent anabolic effects and unique neuroprotective properties, with recent clinical interest shifting toward oncology. This article compares their mechanisms, efficacy, and safety profiles based on current peer-reviewed literature.
Mechanism Comparison
Both LGD-4033 and RAD-140 function as high-affinity ligands for the androgen receptor (AR). Rather than acting as simple agonists, they exhibit tissue-selective activation.
- •LGD-4033: This compound binds the AR with a $K_i$ of approximately 1 nM. In muscle and bone, it acts as a full agonist; however, in the prostate and sebaceous glands, its activity is significantly attenuated. Crucially, LGD-4033 is not a substrate for 5-alpha reductase or aromatase, meaning it does not convert to dihydrotestosterone (DHT) or estrogen (Basaria et al., 2013).
- •RAD-140: Exhibits a similarly high affinity for the AR ($K_i$ = 7 nM). Preclinical data suggests RAD-140 is exceptionally potent in stimulating levator ani muscle growth while requiring significantly higher doses to induce seminal vesicle or prostate stimulation compared to testosterone propionate (Miller et al., 2011). A unique mechanistic feature of RAD-140 is its neuroprotective capacity; it has been shown to reduce cell death induced by apoptotic insults in cultured neurons (Jayaraman et al., 2014).
Efficacy Comparison
The evidence for efficacy differs in terms of "quality of evidence," with LGD-4033 possessing more robust data in healthy human populations.
Ligandrol (LGD-4033)
In a landmark Phase I clinical trial, 76 healthy men received LGD-4033 for 21 days. The study found a dose-dependent increase in lean body mass (LBM). Participants receiving 1.0 mg/day gained an average of 1.21 kg (2.67 lbs) of LBM. Strength (stair climbing power) showed a trend toward improvement, though it did not reach statistical significance in this short-term, low-dose safety study (Basaria et al., 2013). Subsequent Phase II trials (under the name VK5211) demonstrated significant increases in LBM in patients recovering from hip fractures.
Testolone (RAD-140)
The majority of RAD-140’s anabolic data is derived from animal models. In castrated rats, RAD-140 was found to be more anabolic than testosterone at equivalent doses in terms of muscle weight gain, with only a fraction of the androgenic effect on the prostate (Miller et al., 2011). In human clinical research, RAD-140 has been studied primarily in the context of AR-positive/HER2-negative metastatic breast cancer. A Phase I study (Hamilton et al., 2024) indicated that RAD-140 had an acceptable safety profile and demonstrated clinical benefit (stable disease) in a subset of patients, but these results focused on oncological markers rather than muscle hypertrophy in healthy adults.
Safety & Side Effect Comparison
Neither compound is free of side effects, particularly regarding the hypothalamic-pituitary-gonadal (HPG) axis and lipid metabolism.
| Side Effect Profile | Ligandrol (LGD-4033) | Testolone (RAD-140) | | :--- | :--- | :--- | | Testosterone Suppression | Dose-dependent suppression of total testosterone and SHBG. | Significant suppression of total testosterone and SHBG. | | Lipids | Significant reduction in HDL (high-density lipoprotein) cholesterol. | Reduction in HDL and LDL (dose-dependent). | | Liver Enzymes (ALT/AST) | Generally stable in clinical trials, though case reports of idiosyncratic drug-induced liver injury (DILI) exist. | Case reports indicate potential for significant hepatotoxicity/cholestatic liver injury. | | Hormonal Conversion | No aromatization to estrogen. | No aromatization to estrogen. | | Prostate Effect | Minimal change in PSA levels. | Antagonistic effect on the prostate in the presence of testosterone (preclinical). |
Note on Hepatotoxicity: While LGD-4033 clinical trials showed no significant transaminase elevations, real-world case reports have linked both compounds to liver injury, likely due to the higher doses used by "grey market" consumers compared to clinical settings (Koller et al., 2021).
Pharmacokinetics Comparison
- •LGD-4033: Displays a prolonged elimination half-life of approximately 24 to 36 hours. It shows dose-proportional accumulation in plasma with repeated daily dosing (Basaria et al., 2013).
- •RAD-140: Early animal studies suggested a shorter half-life; however, human clinical data (Hamilton et al., 2024) suggests a much longer half-life in humans, approximately 60 hours, allowing for once-daily or even less frequent dosing.
Summary Table
| Feature | Ligandrol (LGD-4033) | Testolone (RAD-140) | | :--- | :--- | :--- | | Primary Research Goal | Muscle wasting, hip fracture recovery | Breast cancer, neuroprotection, muscle wasting | | Human Clinical Phase | Phase II (as VK5211) | Phase I (Oncology focus) | | Muscle Gain Data | Strong human clinical evidence | Strong animal evidence; limited human | | Neuroprotection | Not significantly studied | Demonstrated in preclinical models | | Half-Life | ~24–36 hours | ~60 hours | | Suppression | High (Dose-dependent) | High |
Research Limitations
- •Lack of Long-term Data: No studies have evaluated the safety of LGD-4033 or RAD-140 beyond 12 weeks in humans. The long-term impact on cardiovascular health and the prostate remains unknown.
- •Discrepancy in Populations: Most LGD-4033 data comes from healthy young/elderly men, whereas most RAD-140 human data comes from post-menopausal women with advanced cancer. Comparison of "anabolic potency" in healthy humans is currently speculative.
- •Regulatory Status: Neither compound is FDA-approved for human use. The "SARM" products sold online often contain inaccurate dosages or completely different substances.
