Survodutide is a dual GLP-1 and glucagon receptor agonist in clinical development by Boehringer Ingelheim for obesity and metabolic dysfunction-associated steatohepatitis (MASH). This review summarizes the clinical evidence.

Overview

Survodutide (BI 456906) is an investigational dual agonist of the GLP-1 and glucagon receptors, developed by Boehringer Ingelheim in collaboration with Zealand Pharma. It is designed to combine the appetite-suppressing and insulinotropic effects of GLP-1 receptor activation with the glucagon-driven promotion of hepatic fat oxidation and energy expenditure, aiming for superior weight loss and liver benefit in metabolic disease.

Clinical Evidence

Obesity (Phase 2)

The phase 2 trial in obesity demonstrated dose-dependent weight loss, with the highest doses achieving mean body weight reductions in the range of 7–10% over 16 weeks, exceeding placebo and comparator GLP-1 agonist effects at comparable timepoints. Survodutide advanced to Phase 3 development for obesity based on these results.

MASH / NASH (Phase 2)

A Phase 2 trial in metabolic dysfunction-associated steatohepatitis (MASH, formerly NASH) reported a statistically significant proportion of patients achieving MASH resolution without worsening of fibrosis, a key registrational endpoint. These results supported advancement to Phase 3 in MASH.

Mechanistic Rationale

The dual-agonist design leverages complementary physiology: GLP-1 receptor activation reduces appetite and postprandial glucose excursion, while glucagon receptor activation stimulates hepatic fat oxidation and gluconeogenesis (the latter counterbalanced by GLP-1). The net metabolic effect is weight loss combined with reduction in hepatic steatosis — a profile particularly relevant to MASH.

Safety Considerations

Adverse events are primarily gastrointestinal (nausea, vomiting, diarrhea), consistent with the GLP-1 agonist class. Phase 3 programs are collecting long-term cardiovascular and liver-related safety data.

Evidence Assessment

Survodutide is supported by active Phase 2 clinical trials with positive efficacy in both obesity and MASH, and has advanced to Phase 3 development. It represents a high-evidence-tier intervention within the metabolic and weight management category.
