Overview
Semaglutide is a long-acting glucagon-like peptide-1 (GLP-1) receptor agonist developed by Novo Nordisk. Approved by the FDA under the brand names Ozempic (for type 2 diabetes), Wegovy (for weight management), and Rybelsus (oral formulation for diabetes), semaglutide has become one of the most extensively studied and widely prescribed metabolic peptides in modern medicine. Its development represents a milestone in peptide drug engineering, achieving once-weekly dosing through structural modifications.
Mechanism of Action
Semaglutide is a modified GLP-1 analog with 94% sequence homology to human GLP-1. Key modifications include an amino acid substitution (Ala to Aib at position 8) that provides resistance to dipeptidyl peptidase-4 (DPP-4) degradation, and the attachment of a C18 fatty diacid chain that enables strong binding to albumin, resulting in a half-life of approximately 165 hours. GLP-1 receptor activation stimulates glucose-dependent insulin secretion, suppresses glucagon secretion, slows gastric emptying, and promotes satiety through central nervous system effects on appetite-regulating centers.
Research Highlights
The SUSTAIN clinical trial program established semaglutide's efficacy in type 2 diabetes, demonstrating HbA1c reductions of 1.4-1.8% and weight loss of 4-6 kg. The STEP trials for obesity demonstrated mean weight loss of 14.9% at 68 weeks (STEP 1). The SELECT cardiovascular outcomes trial demonstrated a 20% reduction in major adverse cardiovascular events. Research has also shown benefits in fatty liver disease (STEP 5, 8), and ongoing studies are investigating applications in addiction, Alzheimer's disease, and sleep apnea. The oral formulation (Rybelsus) represents the first approved oral GLP-1 agonist.
Dosage & Administration
Injectable semaglutide (Ozempic/Wegovy) is administered once weekly subcutaneously. Dosing begins at 0.25 mg weekly for 4 weeks, escalating to 0.5 mg, then 1.0 mg (Ozempic) or up to 2.4 mg (Wegovy) as tolerated. Oral semaglutide (Rybelsus) is dosed daily at 3-14 mg on an empty stomach. For research use only (RUO) — this information is provided for educational purposes only.
Safety Profile
Common side effects include nausea, vomiting, diarrhea, constipation, and abdominal pain, typically diminishing over time. Contraindicated in patients with personal/family history of medullary thyroid carcinoma or MEN2. Boxed warning for thyroid C-cell tumors based on rodent studies. Rare reports of pancreatitis, gallbladder events, and acute kidney injury. Diabetic retinopathy complications have been observed in patients with rapid glycemic improvement. Not recommended in pregnancy.
