Overview
Retatrutide is a first-in-class triple hormone receptor agonist targeting GLP-1, GIP, and glucagon receptors simultaneously. Developed by Eli Lilly, it represents the next frontier in metabolic peptide therapeutics, moving beyond dual agonism to harness the complementary effects of three key metabolic hormones. Retatrutide is currently in Phase 3 clinical trials and has generated significant interest for its potential to achieve weight loss results exceeding even tirzepatide.
Mechanism of Action
Retatrutide's triple agonism produces effects beyond those of dual GIP/GLP-1 agonists. GLP-1 receptor activation suppresses appetite and improves glycemic control. GIP receptor activation enhances insulin secretion and adipose tissue metabolism. The addition of glucagon receptor agonism promotes hepatic fatty acid oxidation, increases energy expenditure, and may contribute to greater fat mass reduction. This triple mechanism addresses multiple metabolic pathways simultaneously, potentially explaining the enhanced weight loss observed in clinical trials.
Research Highlights
The Phase 2 TRIUMPH trial demonstrated mean weight loss of up to 24.2% at 48 weeks at the 12 mg dose — the highest weight loss reported in any Phase 2 obesity trial at the time. Retatrutide showed significant improvements in glycemic control, blood pressure, and lipid profiles. Subgroup analysis indicated particularly strong responses in male participants. Research has also shown improvements in markers of non-alcoholic fatty liver disease (NAFLD). Phase 3 TRIUMPH trials are ongoing, investigating long-term efficacy and safety.
Dosage & Administration
In clinical trials, retatrutide is administered via subcutaneous injection once weekly, with dose escalation from 1 mg to 12 mg over 16-24 weeks. The titration schedule is designed to minimize gastrointestinal side effects. For research use only (RUO) — not yet FDA-approved.
Safety Profile
Reported side effects are consistent with the incretin class, including nausea, diarrhea, vomiting, and constipation. The glucagon receptor agonism component may contribute to additional effects on hepatic glucose output. Long-term safety data is being collected in ongoing Phase 3 trials. Not yet approved by the FDA for any indication.
